"After fulfilling all other specific provisions, I give, devise and bequeath _____% of the remainder (or $______) of my estate to the Friends of the Pittsburgh Fisher House, a charitable corporation. Tax ID: 46-0881237, currently having offices at Friends of the Pittsburgh Fisher House, 3945 Forbes Avenue, Box 240, Pittsburgh, PA 15213."
